COMPENSATION: $130,000 BASE - TOTAL Potential $143,000+ Full Benefits

Partnering with a prominent value-based care organization to recruit empathetic and skilled Advanced Practice Providers dedicated to providing top-tier, home-based primary care services for the elderly Medicare population in Las Vegas. These providers will offer care directly in the homes of patients, effectively eliminating access barriers and enhancing health outcomes for marginalized communities.

Rate: $140,000

Qualifications

• Active and unrestricted NP or PA license in Nevada
• BLS/ACLS certification
• Experience in geriatrics, internal medicine, or family medicine preferred
• Comfortable managing patients with multiple chronic conditions
• Ability to lift and transport up to 20 lbs. of equipment
• Strong communication, empathy, and clinical decision-making skills
• Reliable vehicle and valid driver's license required

Responsibilities:

• Deliver primary care and consultative services to members in their residences
• Collaborate with leadership to identify and arrange visits for patients requiring home-based care
• Work closely with Medical Assistants and care teams to ensure and enhance quality, continuity, and patient contentment
• Maintain precise records of all services and patient interactions
• Engage in quality enhancement initiatives and continuous care coordination
• Provide healthcare services within a designated geographical area using dependable transportation
